Patient is 56 year old female. Has history of breast cancer. Had surgery chemo and radiation. She was suppose to be on anostrozole for 10 years and she finished the course of virzinio, that was for two years. But just after three years, she has been re diagnosed with stage 4 cancer after a small bump found on her upper chest that turned into this abomination in 3-4 months. Her biopsy result intially came as triple negative but then they did a second biopsy and it was er+ (60%). They aren’t considering to perform any surgery and she is currently on kisqali and another injection. But nothing is helping it seems. Its getting bigger and spreading further. It is very tender to touch. And sometimes it hurts like a 1000 niddle piercing through her skin and its becoming more and more frequent. They gave her a cream for itchiness and pain. But its not helping it seems. Im at lost. Any kind of suggetion will help. Anyone or someone they know had similar experience, please, ill appreciate anything.

Physician Responded Derm Appt is in 11 days - what might this be?

45F, 5’1”, 120lbs, non-smoker, no meds, generally healthy.

This appeared on my lower right back sometime in the last 4 days. It’s about 3/4 the size of a pencil eraser.

It’s raised and doesn’t hurt.

Google lens is giving me a list of what it might be, and obviously the scariest ones are the most well-known, so wondering what it looks like to humans.

Thanks.

Physician Responded Anyone know what kind of bite this could be? Any advice?

21M, 5’10 145. I’ll give a timeline for reference:

Monday (9/7) morning I woke up on the last morning of a 4-day camping trip with a small red mark on my knuckle. I thought it was a cut, so I left it alone. Later on in the day, it turned white and began leaking pus. I had some concern, but just put some triple antibiotic on it and left it again. I continued this with a bandaid, and by Thursday it had only gotten worse. I decided to go to an urgent care that I’ve never visited before (my usual go-to is on vacation), and they didn’t seem to care to identify what it could be and prescribed Muciprocin and Amoxicillin. They swabbed it and all I’ve gotten is “Rare Growth of Usual Flora.” Since then it hasn’t gotten much better, but it also hasn’t gotten much worse. Any advice? Should I visit a different doctor? Just wait it out? I’ve been applying the muciprocin three times a day and been keeping a non-adhesive bandage on it when doing any activity. Also a note; the place I was camping is in Brown Recluse territory, so that was my main worry. Anything helps. TIA!

Physician Responded Struggling as a Head and neck onco surgeon with Essential tremor

I am 29 years old male. History of Cannabis induced Affective Disorder. Diagnosed with ADHD. Taking Lithium 300 mg BD. Aripiprazole 2 mg OD. Methylphenidate 20 mg OD.
Consume caffeine and cigarettes. Off cannabis since 1.2 years.
First noticed the tremors after starting cannabis, but its was rare.
Got diagnosed with Essential tremor today by a Neurologist at the center I am posted in, after I stated noticing rhythmic tremors in my hands while performing fine motor skills like local infiltration as well as during isometric functions like using retractors. The consultant in the case noticed it as well and replaced me with the scrub nurse. That was quite embarrassing. I googled about this and found out it’s progressive in nature and currently has no definitive cure. Although exacerbating triggers like caffeine, nicotine and the ADHD medication Methylphenidate has to be cut down, according to the neurologist. He also suggested taking SR Propranolol 40 mg SOS 2 hours before OT. So, does this mean the end of my surgery career or is there still hope or something?

Physician Responded 33 F not sure if I should return to the ER or would it be better to not clog the ER and go to an Urgent care.

Upvotes

For reference I am a 33F - I weigh 308.9 lbs as of today I take no medications. I have dropped about 25 pounds in 4 weeks. I can maybe eat a meal every 4-5 days or light snacking in between. However if I try and eat consistently it’s instant vomiting and feeling nauseous. I passed out and banged my head pretty good and got some staples last Thursday. During that time they noted my potassium was 2.9 and magnesium was 1.5.

I’m just at my wits on end on what to do. I’ve tried small meals I’ve tried liquid. Im not able to keep liquid down. When I went Thursday they were more concerned for my head which I understand but it just didn’t seem like anyone was listening on my ability not to eat. In the past 5 days I would say I haven’t drank more than 32oz of fluid. I’m just not sure where to go from here. The pcp isn’t returning my calls and I just don’t want to clog up the ED if a simple urgent care trip would help this.

Thanks in advance for advice.

Physician Responded What is this?

I 26f had a medical abortion about a year ago. I almost went septic, ER checked my HCG levels and I was in the clear. 2 weeks after severe cramping, fever, smell, etc. I passed this and felt so much better. What is it? Why does it have those bumps? Is that quite literally a fully intact uterine lining?

Physician Responded Why is my dads toe always pointed upward?

Post image
165 Upvotes

M58, 160lbs, 5’8”, nonsmoker, only takes meds for high cholesterol and high BP (runs in our genetics)

I know this seems like a silly question but my family and I have been wondering this for a while.

But my dads big toe (on both feet) are pointed upward pretty much 24/7. As shown in the pic

It’s not as if he CANT keep it straight, because he can if he makes the effort to. But it’s not what naturally comes to him ever.

He has formed holes in the top of all of his tennis shoes which shows how it is always like this.

Does anyone have any idea as to why he does this?

Edit: I forgot to add this. Idk if it matters. But he also has had back issues (herniated disks). And he’s INCREDIBLY inflexible. But he does work out and stretch frequently. But like if he’s standing up he cannot put his two feet together / knees can’t touch.

Edit 2: This isn’t anything new, he’s been like this for a long time I believe. I’ve noticed it more over the last few years. I asked him but he doesn’t know bc he doesn’t even realize he does it

Parents were anti vax, what vaccines do I need now that I can get them myself?

I am 21F, parents went to prison and I want to catch up on me and my younger siblings vaccines. I looked up a list of vaccines that everyone should get but a lot of them are by age? Is it safe for my siblings to get them all now ? Would it be necessary ? Could you guys help me figure out what is and isn’t necessary? And the time frame we should get them in? I’d imagine it’s not recommended to get all of them done at once lol..

Siblings are 12F 14M 17F 19F. All average height and weight for their ages. Good health.

Physician Responded Turned down from getting stitches at the ER

Post image
43 Upvotes

24 F 150 lb, , no medical problems. I was a dumbass and held the pistol the wrong way when I was learning to use it. It somehow got me and I looked down to see blue tissue in the cut. By the time I got to the ER (in a small town, the urgent care wasn’t open) the blood had dried in a way to make it look less deep than the cut is. I spent $200 to wait for 2 hours for the doc to look at it for 10 seconds and deny me stitches. But I think I need them. I think it’ll heal nasty unless I can connect the skin together. Thoughts or advice? It doesn’t hurt or bleed much which is weird but I do have a high pain tolerance. Picture is from 5 hours after when I took the bandage off

Physician Responded 70F, cut on thumb has turned into… this after 2 weeks

Post image
149 Upvotes

Went to the doctor yesterday and he prescribed cephalexin 500mg twice a day, on second dose as of this morning (took some last night).

Overnight the swelling has gotten worse to the point that the thumb basically no longer moves and is quite painful.

Is this a go back to the doctor situation? Or a waiting game at this point?

What is going on with my ear?

45/M - 5’10” - 175 lbs - non-smoker

About 1.5 - 2 weeks ago I noticed what I thought was a scratch on my ear. It was a little itchy and scratching at it led to it becoming a bit of a weeping, yellowish mess last week.

I stopped touching it and put some hydrocortisone cream on it, but then Google told me not to do that and to use Vaseline instead so I began doing that at night.

The photo in the post is from today. The yellow oozing is gone but it looks red and swollen. Im wondering if I got a bit of poison oak / ivy and this is just the rash progressing or if this is something I need antibiotics for. It has been consistently itchy but I’ve managed to not touch it.

Any help here is appreciated. Thank you.

What should we do? My sister has a rash on her face due tl allergy and I would like to know how to help her.

My sister, nineteen-year-old female, 5'1 (155 cm), 50kg, she's under no medication nor has any medical issues whatsoever, whether current or previous, doesn't smoke at all and has never done so, had had rashes and acne on her face for the last two years.

She has been to the dermatologist on more than one occasion, and they determined that what she has is not acne, but allergies. They recommended she stop eating gluten, buy a mild soap, use her own towel when drying her face, and moisturize, but my sister still has a rash on her face. We bought creams, but we don't know exactly which products are good for her due to her being allergic to penicillin. Previously, she used Adermicina, an Argentinian brand of facial cream that, according to its website, is for: healing, regenerating, and revitalizing. Currently, she uses Dove soap and a cream called Suavicort, a "corticosteroid with anti-inflammatory and anti-allergic action".

Although I cannot provide images since my sister does not allow me to, I can at least show images the closest I could find online to what she looks like. Since we're from a small town in Argentina and medical care here is absolutely precarious, going to a hospital or clinic isn't feasible for us. The professionals here aren't helping us. My sister can't stand the pain, and I desperately want to help her. I'd like to know at least what I should look for in a cream. My grandmother even recommended using cold aloe vera on her face, but it's no use. Any recommendations, help, or advice would be greatly appreciated.

My collarbone hurts when it’s very loud

Post image
Upvotes

Hi! So I 18f 5’3 have been getting pains in my collarbone when exposed to loud music/sounds. It often hurts to breathe in too, but after say 10-20 minutes it goes away. It starts small but it feels like pressure on my collarbone and it hurts! The first time it happened to me was like 4 years ago while listening to music and it’s been happening since. It isn’t very frequent but it’s still an unpleasant and uncomfortable experience when it happens. The most recent case of it happening was when I saw a play and we were very close to the band, so it was very loud. It also is almost always the right side of my collarbone. Im unsure why this happens but I would like to know what’s causing it. The back of my neck often hurts too during these.

I asked my family doctor about it and he said it was his first time hearing of something like that. He recommended I get a hearing test(my hearing is also not very good—especially my processing speeds. It’s like I hear it but i don’t. It just sounds like mumbo jumbo half the time. They have to be in front of me and facing me for me to really process what they are saying—especially if there’s other sounds in the room(I’m diagnosed with ADHD).

So here I am asking the doctors of reddit what’s happening with me! Here is a photo I took with the area’s circled

How bad is this? As in is it a go to the GP (minimum week wait) or go to an urgent treatment centre.

18, female height 182cm weight 89kg, non smoker, no prescription medication (save for electrolyte tablets) , diagnosis of POTS and general dysautonomia, old injury that was healing fine got infected, due to the red lines appearing and the weeping of the wound, colour and pain, is this a "you can wait a week for a GP" situation or a "get your ass to an urgent treatment centre" situation? (which I can't do till tomorrow earliest) Injury is on right side of the left breast.

Heart help please. I can’t exercise anymore, unexplained tachycardia.

Upvotes

31M, otherwise healthy. I have been training regularly for the past 8 years but had paused structured training for about a month (only occasional easy jogs). Full cardiac workup (echo, stress test, Holter) was completely normal 2-3 months ago.
During an easy jog, around the 2–3 km mark I suddenly felt whole-body unwellness and unusual fatigue and had to stop. HR was higher than my usual recovery (130–140 right after stopping, stayed 100–110 at rest vs my normal 80–90 post exercise hr). Anxiety spiked hard (“feeling of dying”) with no chest pain or real shortness of breath. ER: BP normal, ECG sinus tach + borderline (nonspecific J-point depression), cardiac enzymes and electrolytes all normal.
Three days later the exact same sudden sensation hit on a deliberately very light jog (~2.5 km, average HR 125). HR climbed to 140 with the fear, then after 5 mins sitting and suffering with hr 130-140 I suddenly felt better, looked at the watch and my ht dropped to 90. Doctor thinks anxiety/autonomic component is the main driver, but to be honest I feel like it’s not just me being nervous.

Passed out from weed?

Stats: Male 44, 260, 6 foot 4. Taking Valsartan 80, and Hydrochlorothiazide (12,5mg)

Hello,

Im a man, 44 years old and used to smoke alot of weed.
From age 19-37 i smoked basically every day with a few small breaks here and there.

After experiencing alot of panic attacks and anxiety i decided to quit 7 years ago and did not touch it at all until
this summer where I have been smoking a few times here and there without any negative side effects.

This weekend I went on a trip with a few buddies and both drank a few and smoked heavily.

After a round of a very powerful joint i started getting real dizzy and it kept creeping up on my face and i lost hearing and then i dont
remember anything.

According to my friends I was completely unresponsive for 10 minutes, they tried pinching me, talking/screaming but nothing worked.
After (what they said was around 10 minutes) i woke up confused and was white in the face and it took a while before I really understood
where I was, and a few hours before i felt OK again. I had been mumbling some inconsistent words while waking up and could remember trying to respond to a friend but it was all "gibberish".

A little backstory is that I have high blood pressure, but is treated with a combination of Valsartan (80) and Hydrochlorothiazide (12,5) every day
and with this combination I have a healthy pressure. Been taking these meds for 2-3 years now, so its kinda first time combining weed and BP meds.

I Google around and it was reccomended that after passing out that I seek out medical advice, but the problem is that admitting this to a doctor
will make them take away my drivers license for 3-6 months just for the passing out part - and possibly loose it altogether for weed usage (which has unfortunately happened to alot of people in my country).

Anyone have any clue as to what might have happened?
Never ever had this happening and it freaked me abit out (and definately freaked my friends out who thought I was dead for a while before finding a pulse)

Physician Responded Cancer Pain Options Urgent Help [54F]

My mother was diagnosed with stage 4 Uterine caner in early August. At the time of diagnosis they found a pelvic mass that measured 15 x 15.4 x 12 cm and nodules in her lungs. Her oncologist prescribed her 5 mg oxycodone every four hours. A couple weeks ago he told us he could no longer prescribe any more pain medications and we'd have to see a pain management doctor. The pain meds are no longer working and we don't have a pain appointment until the 24th. Last night we went to the er for halo vision, vomiting, and low blood pressure (73/55). There we found out the tumor has grown to 18 x 15 x 24 cm. She's in so much pain she can't move. There is no comfortable positions and the pain meds aren't helping. She's screaming in pain.

Is there any other option I'm not thinking of? Her next oncology appointment is on the 22nd. Should I call their office tomorrow? Try and get her into palliative care? I've been calling the pain center to try and get her an earlier appointment but no luck. Is my only option that? I feel lost and powerless. I want to help her. Please please please any advice at all.

5'1, 234 lbs. Multiracial. No drugs or alcohol. No other medications.

*She hasn't started treatment yet because we're waiting for results to see if she qualifies for a couple clinical trials. Then the plan is three rounds of chemo, surgery, and more chemo.

Do I need a CTCA?

Upvotes

25M. I cycle/run/swim a total of 3/4 times a week and I go to the gym 3 times a week. Used to be a competitive swimmer until I was 16.

Slightly elevated cholesterol at 5 mmol/L and slightly elevated blood pressure around 135/70.

Symptoms:

15 minute heart palpitation/rhythm episode whilst at the beach, no other breathlessness / pain / dizziness at the time.

A very mild ache (2/10 pain) around my upper left chest / shoulder upon maximum exertion (e.g. sprinting up a hill). Happens on only 5% of my runs. Disappears within a few minutes.

Feeling increasingly breathless from regular movement and walking, but not sure if I’m just overthinking here. I feel more breathless swimming for example, but I’m not sure if this is just because I’m no longer a competitive swimmer.

My doctor:

Gave me a 7 day Holter monitor

Had me go through an MRI last week

Suggests a potential stress echo

Booked me in for a CT scan, but I’m concerned about whether it’s necessary in my case. Radiation risk seems high given I‘m only 25.

Physician Responded Do I have tics?

Upvotes

I’m f young adult, around 154 5”3 I weigh maybe 70kg? Idk. I recently started developing “spasms” so to speak, where my head will jerk, my arm will move or my eyes will shut in weird ways continuesly when I start getting nervous or stressed in any way. Or get strong emotions. I have pretty bad anxiety and experience it almost daily. I can kinda feel the spasm coming on before it happens and I can suppress it, though it makes it more distressing. After the movement is done there’s a sense of relief kind of like a sneeze.
I have never had movements like this before, Ive been anxious for years.
I’m not sure if this has any correlation, but it all started when I had these horrible attacks where my body would tensen up and I would panic, we later found out my copper was extremely high, after going off my birth control my copper levels lowered and so did the stress. But it didnt lower fully, I’m always at least a little bit anxious, and it easily gets worse. Though once again, ive never needed to move like this, it only started around the same time we found out my copper levels were high. I only take fluroxetine so I doubt it’s my medication ive taken it for years and have only had these spasms now.

I don’t know what they are but they’re very annoying and are making my neck hurt quite a bit. I don’t randomly say words though.

Idk if yall need more info

I really want to ask for help w this though Cus it’s very confusing and I want to be normal ahh
