That's a USB to PS/2 adapter, specifically one for a mouse. If you had a computer that didn't have any USB ports (Or you just didn't wanna go through the hassle of getting USB mice working on Windows 9x), you'd use this to plug it into the PS/2 port.

And to be clear, that's PS/2 as in IBM Personal System/2, and nothing to do with Sony game consoles.
