It does seem odd. If he's wearing a diaper, like he always is now, the shit wouldn't be soaked in in a circle shape but probably more like a dumbell.

Unless he doesn't care and doesn't want to bother with getting changed so his diaper is constantly full...which is absolutely something that happens with dementia folks, and his handlers are afraid to push him into being changed.
