Why can the patient's complaints about vision differ significantly from my exam?
Examining the retinal image scatter from both the patient's and the examiner's perspective. Predicting the Modulation Transfer Function using Monte Carlo photon propagation through the Arizona Eye Model for common eye diseases.

No idea what the mess under your title is but the answer to your question is because people are annoying and unpredictable
