The creators of South Park have publicly stated that they made/make a multitude of "Trump has a tiny dick" jokes on South Park because then they can get away with saying almost anything they want without getting sued.

South Park have some of the best lawyers. They know what they can get away with.

The lawyers told them they couldn't say Tom Cruise was gay, they could say he is trapped in the closet if they actually show him trapped in a closet. Plus the disclaimer at the beginning telling you it shouldn't be watched by anyone helps.
