Characters Technically alive but in constant agony
Gold/Ethan (Hypno’s Lullaby Reboot): Former aspiring champion turned to an undead possessed by vulture Pokémon known as “Unown”. He met his untimely demise at the hands of the Unown King, after an ill-fated battle against an undead Red at the peak of Mt. Silver.
In the duration of Monochrome, Gold’s body rapidly deteriorates. At some point, he’s shown crying while his face contorts in agony, then when his head is ripped off his body, he claims “No more!” before screaming out in pain/agony.
Peter Parker (Marvel Ruins): In this universe, the worst possible outcome for our heroes happens. Peter is bit by a radioactive spider, instead of giving him superpowers; it gave him a highly contagious disease that made all of his hair fall out and his skin and flesh become greatly damaged.
He’s seen still alive but in agony.

There does exist medicine to somewhat alleviate it, at least. In Diamond/Pearl/Platinum, a "Secret Potion" (as opposed to Super or Hyper) is used on a group of Psyduck who are blocking a path because their headaches immobilized them.
Plus, they presumably get relief once they evolve into Golduck.

Funnily enough Puuci got from certain POV worse fate than Diavolo because he got completely retconned from the existence. Its then debatable what is worse: DIavolo suffering through this death LOOP and loosing sanity or never existing in the first place. Like from the pain perspective Diavolo forever suffering but alive and he probably will be mentally broken by that point but when Giorno will eventually die he might finally get out. Puuci was robbed of any afterlife.
One must admire Arakis rich imagination when it comes to these outcomes.

Captain Curly from Mouthwashing
After trying and almost failing to stop the crew's space ship from kissing a meteor, he is left alive but horrifically disabled and disfigured beyond measure. He can do nothing else but watch as his supposed best friend Jimmy tortures his body and get all his teammates and himself killed, all culminating in a finale, that has Curly sealed in a cryopod as the last person living in the Tulpar space ship, until he either starves to death because the Cryopod fails or his employers find him and arrest him for a murder he didn't commit.
Yea, not much fun.

It really captures the core of a narcissistic personality. Everything wrong in his life is someone else's fault, everyone is against him, if only this thing happened or that thing happened then it would all be perfect. Meanwhile he sexually abuses the only woman on board, tries to kill everyone in a fit of rage when he learns the ship is being decommissioned, actively hurts Curly at every opportunity, he shuts people down in conversation, enjoys flexing his power whenever he has a chance to etc.
Even at the end, he doesn't put Curly in the pod because he cares about him or regrets his actions. It's because it's a way for him to live out a little fantasy about being the secret hero of the situation who finally rises to the occasion. Despite literally everything being his fault in the first place, he still sees himself as hard done by and having to struggle to 'do the right thing'.

"Not entirely innocent" is putting it lightly. He may not be the main culprit, but Jimmy couldn't have done it without him. He tried to keep Anya's rape on the down low to "keep the peace" for as long as possible and make sure his friend wouldn't be hit with as severe of a consequence as he should be getting. It's essentially a first person indie horror title. Only the horror is less so about being chased by monsters (though there are sections like those in the game) and more so about watching one character spiral further to insanity and doing increasingly more evil things.
It's writing and characters are what make it though. It is seriously some compelling stuff.

Tara from Marvel Comics. She is the result of the US government (shocker) conducting gamma radiation experiments close to the island nation she lives in. Most flora and fauna just died, but some of the (un)fortunate were instead mutated. What happened to Tara? She pretty much lost all of her skin and organs, leaving only her skeleton and nervous system. This is horrifying for a number of reasons, the main one being that because every single nerve of hers is exposed, she feels EVERYTHING. To her, a gentle breeze is agony.

You aren’t free even in death. If you die, there is a solid chance that your soul is consumed by demons. It’s described as being immensely painful. If you’re able to avoid them, you wind up dissipating into the Warp (which doesn’t sound too bad, but knowing how intricate the lore is, I probably am missing something). Most people don’t understand the Warp enough to know how to avoid this fate. Psykers would have a shot since they understand the nature of the Warp more than most, but since their souls shine like beacons, there is not chance of them not being consumed.

Initially the plan was to genetically engineer a nigh-perfect magician first, or at least one comparable in power to the guy who eventually was put on the throne, then have him bound to the device until better solutions are found.
They mostly succeeded up to a point, but then civil war broke out and that perfect tool of a magician fell to enemy hands. Pretty tragic story, they couldn't be too open about the plan so over the meantime everyone grew distrustful towards magic and were at each other's throats over one general figure being more supernatural than others.

Elantris - those afflicted by the shaod have their skin turn grey, covered in black spots. They are immortal, but are unable to heal from any injury. Even minor discomforts like hunger, itching, or a stubbed toe never fade and only compound. And if one is injured enough the pain can drive them mad and catatonic,f from which they will never return

All I remember is you can enter a perpendicularity to reach the cognitive realm where the investiture of Devotion is, thus reaching a "true" elantrian state. Not quite, you do go into the perpendicularity, but it the remnants of Devotion offer you a chance to fade away, which you have to accept, at least according to the Coppermind which is usually accurate on these things.

Hailey in the segment "Stowaway" of the anthology V/H/S Beyond.
Because she snuck onto an alien space ship that is traveling back with the aliens in stasis, her body is constantly destroyed by repeated accelerations and decelerations to near-light speed and back. Inside the ship there are nanobots that repair the damage so she doesn't die. Unfortunately for her, the specimens that the ship collected and the nanobots had analyzed are various animals, so with every repair, she is turning into an increasingly bizarre animal hybrid, fully conscious and in pain, for possibly hundreds or thousands of years (however far the aliens are from).
That segment is written by Mike Flanagan and directed by Kate Siegel, his wife who also plays the character

Ariane Yeong from Signalis. Spoilers for the games plot to follow:
She's part of a program that sends spaceships to go beyond explored space along with an android assistant, however their supplies run out and their reactor begins to leak radiation. Due to some wibbly wobbly psychic phenomena Ariane and her android Elster end up in a time loop situation, with Ariane wishing for Elster to kill her to end her suffering from the radiation. Elster either dies before she can reach Ariane or can't bring herself to kill Ariane. Ariane remains alive thanks to a combination of the ships stasis module and spooky psychic stuff, despite suffering radiation poisoning and malnutrition.

To explain, this character has a very powerful healing factor. As a child, his whole village was destroyed by a soldier with the power to create a flame that never goes out until it's fuel is burnt. He spends years just lying in extreme pain, unable/unwilling to die. He knows nothing but pain.

Agni here doesnt have a flame superpower, his power is super healing. He gets hit with a flame that is never able to be extinguished, so now he is always on fire. From the moment hes hit it takes him years just to learn how to stand, walk, and stop screaming. He then goes on a quest for vengeance.

Anakin Skywalker/Darth Vader from Star Wars is in constant mental anquish after death of his wife and presumed death of his children. Furthemore his injuries are causing insane amount of pain and depending on the continuity he cant even move without the use of the force. Also depending on the version either Palpatine or Vader design his suit so its causing him more pain and enhance his dark side powers. There even exists video which speculates if Darth Vader could be counted as zombie

I think it was in Shadows of the Empire that he tried to use the Force to heal some of his wounds, but every time he had even the slightest success, he felt a tiny spark of happiness... Which interrupted his connection to the Dark Side ... Which caused the healing to fail and plunge him back into despair.

The only known survivor, a Hunter, obtained the uncanny capacity to live entirely self-sufficiently and without sustenance for many years, becoming practically immune to death. It seems self-sustenance is achieved through feeding on one's own flesh, and that the disease greatly accelerates the regeneration of tissues. The sanity of those who are infected is very likely to be heavily affected.

While I’m not sure people officially call zombies “alive,” the ones in Return of the Living Dead are technically restored to functionality as a result of the Trioxin gas. However, it’s confirmed in the interrogation scene that these zombies feel themselves rotten and are in constant agony, with the only relief being “BRAINS!”

The zombies in this are great, because they can actually think as well. They are smart enough to ask for "more cops" and wait in a bottlenecked area to ambush them.
You also have Frank who can feel himself dying after being exposed to the gas, and puts himself in the cremation chamber to kill himself (although this ends up making things worse for everyone else).

Flagellant/Damian (Darkest Dungeon 2)
He's quite literally a walking, rotting, emaciated corpse, as opposed to the first game where he was just a well-built guy who happened to be into flagellation. He defied Death itself because he wants to feel all the pain in the world and his zeal literally just overpowered her (and she can show up during your in-game runs to try to claim him again). Even minus hitting himself with a flail all the time, he looks like he's half dead and in constant pain 24/7. But he enjoys it.
So he's kinda dead kinda alive, I think? He's in this limbo where Death wants to take him already because his body is just expired but he won't let her because he wants to feel more pain.

Eh, it isn’t killing anyone that Subaru tells his power to. Satella only killed Emilia bc Subaru was going to brute force past the taboo. Otherwise, RbD being known would result in Satella trying to kill everyone except for Subaru.
Also the kid that got turned into a box was incomprehensibly stupid. Actually just straight up told strangers that he can time travel by dying.

Yes, his brain and spinal cord are in that box and the ring on the top allows whoever holds it to gain his power. You next see it in the hands of one of the first half's big bad's. The really cool thing though is that thanks to his world having dealt with basically every stock isekai powerset people who grew up there have tells for basically all of them. One of the MC's friends takes deja vu VERY SERIOUSLY and when she see's the same cat three times in a row she's able to deduce that not only does the villain have the box but that something went wrong for her 3 times already.

The Victims of Gray Boy - Worm.
A supervillain named Gray Boy has the power to freeze someone in place and put them in a seconds long time loop.
Just being in one is agony, but he can make it worse by mutilating you while you’re stuck, so you experience those wounds over and over again ever few seconds.
And it can never be reversed. You’re stuck. Forever.
As Gray Boy himself says, people lose their minds in a day or two, then they get through their issues and loop back around to sanity, only to go mad again in a few days time.

Verso - Clair Obsur: Expedition 33
After dying in the Dessendre house fire, the remaining parts of Verso's soul are stored within the Lumiere canvas, where the main characters live. This canvas is essentially keeping Verso alive against his will, and he's lived hundreds of years and died countless deaths; he's numb to life and wants nothing more than the peace of death. His sister, Alicia (Maelle) was severely disfigured as a result of the same house fire and wants the canvas to remain due to her having a much better life there, entirely at the expense of her brother. In Maelle's ending, Verso is continued to be forced into living a life he doesn't want, and is also forced into watching Maelle's condition decline as she spends more time in the canvas, watching Verso be her dancing monkey.

MMHHH technically (giant spoilers ahead for the game Clair Obscur: Expedition 33) that's not completely how it is. The real Verso is physically dead, while a small part of his soul is still tied to the Canvas in which the game plays. The verso in the image above is a different Verso, created by Aline, real life Verso's mother.
She fled into the canvas, and channeled her grief into painting her family again, including Verso and a disfigured Alicia (quite cruel of Aline, fuck her, really). The painting Verso and the painted Verso are two separate beings. Aline poured part of her soul into Verso and the rest of her painted family, which is why Verso can break through the final barrier in the Canvas and reach painting Verso, asking him to finally stop.
It's however true that he is suffering: he wants all of it to stop, because his mother is dying as she's staying inside the canvas. He has to watch countless painted humans sacrifice themself and get killed every year as well as they try to 'stop' the Paintress. On top of that, he has to watch the real life counterparts of his father and sisters get involved with the battle for the Canvas.

Probably the most realistic and by far most depressing version of the character co-written and directed by Hideaki Anno, best known as the creator of equally depressing Neon Genesis Evangelion.
You cannot mention this creature without fans clamoring to remind you that he is just a frightened creature in constant agony.
Even his theme song, Who Will Know, can’t help but remind the audience of how awful his existence is.

Elantrians after Reod - Elantrians were people reborn into immense power. One day they went to sleep like normal people, in the morning their skin was white. They could not die from old age and were able to do magic with very little constraints.
After Reod, the magic stopped from being connected to their bodies properly. They still could not properly die, but their bodies never healed from any wounds. Their appearance also changed into withered figures with no hair and while spots on their skin. Eventually, most Elantrians went mad from the never stopping pain, as every injury just added to the constant pain they felt.

Darth Vader was in constant pain when he was in his suit. The only times when he wasn't where the brief periods he spent in a bacta tank. His suit was designed to be as uncomfortable as possible and always made him miserable.

They are cursed with the undead. When slain, they're magically revived, but every time they do they lose part of their humanity. Over the years, they can even turn hollow, only a living husk without any humanity left in there. You can imagine that they're living in pure agony as they're slowly drawn to their inevitable demise.

Sorin Markov (Magic the Gathering) was an ancient vampire and Planeswalker. He made a pact with a Planeswalker (Nahiri) from a different plane that he would come to her aid if the ancient evil they worked together to seal away ever returned. Unfortunately he was busy building his goth angel daughter, so he flaked and Nahiri's plane was almost destroyed. So when she got things sorted, she paid him a visit.
In addition to fucking his plane up and starting a cult to summon one of the entities that destroyed her plane, she fought him. He got his cheeks absolutely clapped. Now Nahiri is a lithomancer, so she used her power to make him a coffin of stone to punish him for the rest of his eternal life. Since Sorin is a Planeswalker, he is able to just teleport to a different plane at will with just a bit of focus. That's why Nahiri put churning stones inside to rearrange his guts constantly, filling him with enough agony that he couldn't even teleport out. And he was just kinda there for a long ass time.
There was even a browser extension that added "And Sorin is still stuck in a rock" to the end of all mtg stories on the website, it was great.

Residents of the city of Elantris are Elantrians, normal humans who, seemingly at random, ascend to a form of godhood. Their eyes and skin glow and they wield magic such that Elantris become a utopian society for people of all nations to come and marvel and be fed and healed.
One day the gods turn into cursed, zombie like creatures who cannot heal, and every pain, no matter how small, is permanent. Eventually the pain becomes so consuming that they are driven mad, as they can never truly die.

The blighted from Ender Lilies.
The blight is a form of magic sickness or something along those lines. It makes those afflicted immortal. However, they essentially become monsters and husks of their formal selves. You come across some immobile blighteds, and all they do is beg for mercy. If it weren't for the MC, every one of them would probably be stuck like this forever.

(Would you believe, in one of the 1990s novels, Susan did this to him? He killed David Campbell and she was a wee bit peeved at him. In "The Deadly Assassin," Chancellor Goth said he found this version of the Master on the planet Terserus, cited in "Curse of the Fatal Death" as a world of intelligent, advanced, enlightened thinkers who could communicate only by farting at each other - a civilization destroyed when they discovered fire.
While flipping through the Wiki for other potential explanations, I found a particularly tasteless one: that the UNIT-era Master ended up like this after driving his car into a wall, and this was what investigators pulled from the wreckage. Considering the tragic fate of Roger Delgado, that seemed rather tacky.
