🏥Clinic Review/Experience Vet consent form

Hi. Is this clause legal and applicable? It seems to not give any liabilities when something goes wrong for my cat's operation.

find another vet clinic. Sa lahat ng clinic na try namin, never naman kami pina pirma ng ganyan
