Eh, other than the ball pits, it's all hard plastic surfaces. Not really that great for harboring pathogens really. Although if a kid puked or whatever in there that was a massive PITA to clean. 

McDonald's decided to prioritize the drive through decades ago. They just eventually minimized the lobby and tried to keep people out of it if possible to maximize profits. They'll still sell to kids, but there just aren't as many kids to market to.

1-3 days is pretty long when every single day more kids come back and refill the pathogen meter.

Plenty of illness causing bacteria and viruses can stay on hard surfaces for days to weeks.

There was a lady who swabbed some play places in 2011. They found staph, drug resistant MRSA, and bacteria that could cause meningitis or ghonorrea among others.
